External Data Request Form

Nutrition Services Division

Before submitting a data request with Nutrition Services Division (NSD) Data Management Unit (DMU), please check the California Department of Education (CDE) Food Programs web page and the CDE Data and Statistics web page for reports and specific data available to you. Please note that the DMU does not release student-level data or small geographical unit data. The normal turnaround time for completing data requests is 10 business days and reports are sent in Excel format by email unless you indicate otherwise in your request. All information gathered is kept anonymous. While identifying information is requested, its purpose is to enable us to contact you if further information is necessary to complete your request. All questions are required unless noted "optional," if you have questions or comments about this form, please email the DMU at NSDDATAREQUEST@cde.ca.gov.
